IMMUNIZATION REQUIREMENTS

MMR (measles, mumps, rubella)

Students are required to submit the University Health Form, including a completed medical/health record and current immunization information . This applies to all new or continuing full- and part –time undergraduate and graduate students enrolled in a program of study leading to an academic degree at any public or independent institution of higher education in New Jersey. The State of New Jersey mandates documentation of two MMR (or two measles, one mumps and, one rubella vaccine). The first dose shall have been administered on or after the student’s first birthday and the second dose shall have been administered no less than one month after the first dose. A student who presents documented laboratory evidence of measles, mumps and rubella immunity shall not be required to show proof of vaccination.

Meningitis

The New Jersey Law requires the Meningitis Vaccination (meningococcal) for all new residential students prior to moving into the residence halls. This includes any housing in the summer as well.

Students will not be permitted to move into residential halls until the Counseling, Health and Wellness Center has written proof of your immunization.

Recommended Vaccines

The Hepatitis B and Tetanus vaccines, as well as a tuberculosis test are highly recommended for University students.

Immunization information must be received before University registration. The following documents are acceptable as evidence of a student’s immunization history provided that the type of immunization and the date when each immunization was administered. This documentation will be accepted in the following formats:

1) The university health form may be completed, signed by and stamped by a personal health care provider.
2) Documented on the health care provider's letterhead, official school immunization record, or record from any public health department.
3) A record or an official college affidavit form signed by a health care provider licensed to practice medicine in the U.S. or in any foreign country.

EXEMPTIONS

1) Age: Students born before January 1, 1957 are exempt from the MMR requirements.

2) Religious: If immunization conflicts with your religious beliefs, it will be necessary for you to submit a statement which specifies the religious principle which immunization violates. This statement should come from an official representative of your religion explaining the basis of the religious conflict.

3) Medical: Student must provide adequate documentation from their health care provider (MD, DO, NP) as to why immunization is contraindicated.

A student with religious or medical exemptions from receiving immunizing agents may be temporarily excluded from classes and from participating in institution sponsored activities during a vaccine-preventable disease outbreak or threatened outbreak. This decision shall be made by the institution in consultation with the Commissioner, Department of Health and Senior Services or his or her designee. This exclusion shall continue until the outbreak is over or until proof of the student’s immunization or immunity is furnished. Students born before January 1, 1957 (for MMR only).
